Description: Within this project, two groups of children will be studied: children with autism spectrum disorders (ASD) and typically developing children, including those who are over- reactive to sensory stimulation. In the planned R34 grant, we will pilot test a sensory adapted dental environment (SADE) that has a strong potential to reduce anxiety and behavioral problems among the targeted groups of children. The SADE intervention includes such adaptations as dimmed lighting, exposure to soothing music, and application of a special vest which provides deep pressure sensations that are calming.

Description: The first component of the proposed study will identify the mirror neuron system (MNS) in participants with stroke and damage to the primary motor or premotor cortex by measuring brain activity during action observation and execution in fMRI. The PIs aim to better understand how stroke and motor deficits affect the MNS, and to assess for common patterns of adaptive functional reorganization of the MNS after stroke. The second component of the proposed study will compare the whole brain response between observation of actions that use the counterpart to the paretic and the non-paretic limb.

Description: To address this gap, we will investigate the efficacy of a promising lifestyle intervention designed to prevent pressure ulcers among at-risk members of the [Spinal Cord Injury] population. The resulting five-year study will involve collaboration between researchers from the University of Southern California (USC) and Rancho Los Amigos National Rehabilitation Center (RLANRC), who have developed the intervention based on the results of a qualitative investigation of lifestyle and ulcer risk among adults with SCI. The long-term objective of this project is to identify an intervention option that can enhance the health and life quality of the population of adults with SCI while simultaneously diminishing the heavy healthcare burden that results from the problem of SCI-related pressure ulcers.

Description: Our team used sonography to evaluate the physical mechanisms underlying Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS). We developed and validated a scanning protocol to evaluate the muscles of the male pelvic floor using a transperineal approach. This protocol can be used to evaluate difference among individuals with and without CP/CPPS, as well as identify variations within patients with CP/CPPS. The approach may also be useful for targeting treatments to areas of hyper-contraction (e.g., trigger points) or as a biofeedback tool to train patients in both relaxation techniques and proper exercises for these muscles.

Description: In the proposed study, 440 ethnically diverse elders recruited from a variety of sites in the urban Los Angeles area will participate in a randomized experiment containing a semi-crossover design component. Within either the first or second six-month phase of their study involvement, each subject will receive a lifestyle-based intervention designed to improve a variety of aging outcomes. At 4-5 points in time over an 18-24 month interval, elders will complete assessments of healthy activity, coping, social support, perceived control, stress-related biomarkers, perceived physical health, psychosocial well-being, and cognitive functioning to test the efficacy of the intervention and document the process mechanisms responsible for its effects.

Description: Using the World Health Organization International Classification of Functioning, Disability and Health, the aim of this study was to identify effective strategies for managing urinary and bowel complications resulting from spina bifida. Charts of 210 children between 4- and 13-years-old with spina bifida were reviewed to quantify medical interventions and continence status. Standardized quality of life questionnaires were administered to a subset of participants; child and parent interviews were carried out to examine the experience of living with bowel and bladder incontinence. Practitioners were also interviewed to understand their perspectives of intervention effectiveness.

Description: The study examined from an anthropological, language socialization perspective a practice of “Rapid Prompting” used for communication with children diagnosed with autism. The study employed ethnographic methodology, which included participant observation and video recording in naturalistic environments. The data corpus analyzed in the study consisted of approximately 200 hours of video recorded interactions involving 16 children with severe autism and their parents, siblings, and teachers as they were socialized by an experienced adult instructor into the use of the practice of “Rapid Prompting”; and as adult caregivers and siblings were using “Rapid Prompting” to communicate with children with autism in everyday social situations.

Description: This four year ethnographic and longitudinal study will examine family cultures and family life among 30 African American families and their children in a way that attempts to avoid an overly homogenized description of how urban families at risk manage challenges in daily life as they try to address the health problems of children with illnesses or disabilities. In essence, this is a study of “lives in motion” (Elder, 1998, p.8), building upon earlier research with 20 African American families who have children with special health care needs as well as the health care and school professionals who serve them.

Description: This project examines the beliefs and practices underlying the activities, habits, and daily routines of 18 ethnically diverse consumers with SCI. The intent is to document how personality, lifestyle patterns and choices, and environmental context mutually interact within the individually constructed lives of consumers to influence the development of pressure sores. These results are used to develop a series of applied products, including: (1) a consumer-oriented self-help manual; (2) a set of guidelines for rehabilitation practice; and (3) a lifestyle-oriented occupational therapy treatment model.

Description: In response to this need, we conducted between 1994 and 1996 a randomized controlled trial, the Well Elderly Study, to evaluate the effectiveness of preventive [Occupational Therapy] specifically targeted for urban, multi-ethnic, independent-living older adults. Typically, OT is provided to older individuals to facilitate independence after catastrophic illness or accidents when significant functional impairment or disability is present. However, we reasoned that many of the principles of OT intervention, given their focus on fostering productive and meaningful activity (occupation), maximizing independence, and enhancing function, constituted a potentially effective approach to preventing illness and disability and promoting health in this vulnerable population.
