Special Considerations

Occupational therapy practitioners (OTPs) who would like to apply to receive credit towards certification requirements should apply for Special Consideration before enrolling in any further Lifestyle Redesign courses. Activities that may fulfill certification requirements include coursework, continuing education, or practice experience relevant to Lifestyle Redesign that have been completed within the past seven years prior to applying for certification. Submitting a Special Consideration Form does not guarantee credit towards certification requirements; after submitting, a certification coordinator will review your application and determine which, if any, prior activities count towards certification requirements.

Applying for Special Consideration is not required in order to take courses in the Foundations of Lifestyle Redesign Course Series.

To submit a Special Consideration Form:

1. First complete the Lifestyle Redesign Certification Interest Form and indicate “Yes” to the question “Do you intend to apply for special consideration?”.
2. Upon submitting the Interest Form, you will receive a confirmation email and be redirected to complete the Special Consideration Form.
• For each activity you indicate, you will be required to upload proof of completion and provide course completion dates (among other details, depending on the type of activity indicated) to verify course completion.
• You will be able to save your progress and return to the form by clicking “Save and Return” and following the directions on the screen to return to the survey at a later time. A “return code” will be provided, which is required to return to the survey where you left off.
3. You will receive another confirmation email once you submit the Special Consideration Form. Please allow 4-6 weeks for a certification coordinator to review your application; a decision around which, if any, certification requirements you have fulfilled will be emailed to you from Lifestyle.Redesign@chan.usc.edu.
