Place-based permanent supportive housing (PB-PSH) best practices initiative

Project Summary: The Los Angeles County Department of Health Service’s Housing for Health (HFH) program is collaborating with the USC Suzanne Dworak-Peck School of Social Work and the Chan Division of Occupational Science and Occupational Therapy on best practices to address the complex health and social needs of formerly homelessness adults living in place-based permanent supportive housing. The goal of the project is to produce recommendations and training materials tailored to the specific needs of those working in the context of Los Angeles County.

Methodology/Overview: This project take a Community-Based Participatory Research (CBPR) approach in reviewing documents, facilitating workgroups, and conducting individual interviews to support the facilitation of systems mapping, creation of a best practice guide, and training/technical assistance curriculum for the PSH Integrated Services Program (ISP), which includes HFH Program Managers, PSH Intensive Case Management Services (ICMS), Department of Mental Health Housing Support Services Program (DMH HSSP), Department of Public Health Substance Abuse and Prevention Control Client Engagement Navigation Services (DPH SAPC CENS) in the assessment and care of participants with disabilities. The goal will be to standardize best practice across the HFH PB-PSH portfolio, inform high-end training and technical assistance plans, and establish when/how ISP providers and PSH participants should interface and liaise with specialty service providers such as the DMH and DHS Promoting Health and Preventing Homelessness (PH2) clinical support services teams
