The purpose of this study is to develop and disseminate a toolkit for an evidence-based patient-centered telehealth and shared medical appointment care model for young adults with type 1 diabetes. Previously implemented among a low-SES, racially and ethnically diverse population receiving care at a hospital-based endocrine clinic, we will develop a toolkit of resources to deliver the care model in diverse practice settings, and evaluate implementation and efficacy outcomes in improving access to care, follow-up frequency, and patient and provider satisfaction.
